It is only by applying for unemployment compensation that you can determine if you are eligible.

All claims are processed in a central office in Tallahassee. The One-Stop Employment Centers are only a site to electronically file your unemployment claim.

Our professional staff will assist you with questions regarding the computers or proper formatting in completing the form. Staff cannot advise you on answers to give on your claim.

Out of state employment:

If any employment during the last eighteen months occurred outside of the State of Florida, you will be asked to provide the name, address, telephone number and gross earnings with those employers.

You will receive information about your benefits and call in dates by mail with further reporting instructions within ten days from the date your claim as filed.

  • Social Security Number
  • Current Mailing Address
  • Alien Registration Number (non-citizens)
  • I.D. such as a Driver's License or State ID
• Employment History:
  • Employers' Names or Payroll Company
  • Complete Job Site Addresses & Telephone
  • Dates of Employment
  • Gross Earnings
  • Reason for Job Separation

Other Information

Ex Military:

If you have served in the military in the last eighteen months, you must have DD Form 214, Member 4 form and copies of pay stubs from the federal employer.

Federal Employees:

If you worked for the federal government in the last eighteen months, you must have Forms SF8, SF50 and copies of pay stubs from the federal employer.
